Please note that we don't accept booking or prescription renewal requests through this form. If you would like to book or cancel an appointment, please visit our website at www.thechristieclinic.ca and click "BOOK" or "CANCEL" at the top of the page. Appointments cancelled with less than 24 hours will incur a late cancellation fee. Prescription renewals without an appointment incur a $30 fee per request. If you would like to proceed, please ask your pharmacy to fax us the request.
Medical advice without an appointment is not covered by OHIP. A fee of $65 per issue will apply (the fee is included if you subscribe to our Enhanced Annual Subscription Plan). If you would like to proceed with the request, please submit your medical question below. You will be sent an invoice by email within 1 business day. Once payment is received, your medical inquiry will be forwarded to your physician for review. It can take up to 5 business days for you to receive a response. Alternatively, you can book an OHIP-covered appointment with your physician to evaluate your concern.
Important conditions: This service is only suitable for simple, non-urgent matters that do not require a physical exam. Requests should never be made electronically for emergent, urgent, or serious medical concerns. The doctor will only provide advice electronically if it is safe and appropriate to do so. If your request cannot be handled electronically you may be asked to schedule an in-person appointment. You must be in Canada at the time of your request.
This is for a report copy ONLY. If you would like the doctor's professional interpretation of the results, please book an appointment. Please note: Due to the high volume of results, we do not notify patients of normal findings. We inform you of any abnormal results that require follow-up. However, results can sometimes get lost, so no news isn't always good news. Visit our website for more information on where to access results. You may also book an appointment with your doctor to review results.
