Appointment Cancellations

If cancelling within 24 hours is a frequent occurrence, the clinic will enforce no-show charges. This becomes problematic for the provider and other patients who need our assistance.

Status of Referral

Generally, it takes about 4 weeks to get notified about your referral or test date. Either their office or our office will be in touch. Some consultants wait lists can be as long as 6 to 18 months.

If it's been over 4 weeks, please do the following:

• Check your voicemail or spam folder in your email for a notification from "registration@secure.foremedclinic.com"
• If there is no notification, please fill out the form below, and we will inquire about the status.
• If you are still not receiving assistance, please book an appointment with your Doc

Share a File or Form

If you are sharing a Picture

• Please only take 1 or 2 pics under well-lit conditions.
• Maybe get a good friend to do it for you.
• Upload the file using the link below
• Book an appointment with your Doc to talk about it.
• Give us 1-2 business days to upload the pic to your chart (has to be done manually)
• Photos uploaded without an appointment won't be addressed

Prescription refills or issues

For any prescription-related refills or issues, please book an appointment with your Doc.

If there are prescription issues and it's been less than 1 week since your last visit, please fill out the details below.

Test results

We do not adhere to a "no news is good news policy."

We are partners with patients in their health. We make every effort to review test results expediently and follow up as appropriate. If you are concerned about your results or want to

• review and understand the results
• get a copy of your results

We ask that you book an appointment, and your Doc will be happy to review them with you.

Missed my doctor's call

We try our best to call you on time, but for many factors, it may be delayed. If you missed our phone call, we will call you back later. Please keep your phone out of silent mode.

Rest assured, if you confirm your appointment online, your name is on our schedule.

If we have tried a few times and failed to get in touch with you, our doctors need to move on. Please go ahead and rebook the appointment online.
