Dr. Carruth and the team at Pleasant Valley Dental Care believe strongly in the importance of understanding each patient individually and tailoring treatment options to meet their specific needs. Please carefully read through the steps below to understand our process for establishing you as a new patient at our office:
Step 1: Complete this New Patient Intake Form
Please complete and submit the New Patient Intake Form at the bottom of this page. This short form provides us with the basic information we need to create your patient account, including:
- Contact information
- Basic demographic information
- Dental insurance information, if applicable
Completing the Intake Form does not schedule an appointment. Once we receive it, our team will create your patient account and send you the complete New Patient Welcome Packet.
Step 2: Complete your New Patient Welcome Packet
Once your patient account has been created, you will receive your New Patient Welcome Packet electronically. The packet includes important information that your dentist and our clinical team need before your first visit, including:
- Medical History
- HIPAA privacy forms and acknowledgments
- Office and Financial Policies
- New Patient X-Ray & Scheduling Policy
Please complete all required forms in the Welcome Packet.
We are unable to schedule a comprehensive new patient appointment until the required paperwork has been completed and returned.
Step 3: Decide how to handle your X-rays
A current full-mouth series (FMX) of dental X-rays is required for our comprehensive new patient examination. If you have had a full-mouth series taken within the past three years, you may arrange to have those X-rays transferred from your previous dental office.
If you choose to schedule before we receive your X-rays and appropriate X-rays are not available at the time of your appointment, we will take the X-rays necessary for your examination. Depending upon your insurance benefits and the timing of previous X-rays, some or all of these images may not be covered by your insurance and may become your financial responsibility. IMPORTANT: it is your responsibility to contact your previous dental office and have X-rays forwarded to us.
Step 4: Schedule your New Patient Appointment
Once your required New Patient Welcome Packet has been completed, you are ready to schedule!
Our office will contact you to arrange your comprehensive new patient examination.
Please remember that submitting an Intake Form or completing the Welcome Packet does not automatically reserve an appointment time. Your appointment is confirmed only after you have spoken with our office and selected an available appointment.
Step 5: Your first visit!
At your first appointment, we will complete a comprehensive evaluation of your oral health. This generally includes:
- Review of your medical and dental history
- Necessary diagnostic X-rays
- Comprehensive examination by the dentist
- Evaluation of your teeth, gums, existing dental work, bite, and other oral structures
- Discussion of our findings and any recommended treatment
Please note that there is no cleaning performed at this initial visit. Once Dr. Carruth has determined your tooth and gum health, we will schedule your first cleaning and any additional treatment recommended.
