New Patient Letter

Please correct the errors described below.

WELCOME to our family of fine patients! We would like to take this opportunity to let you know how pleased we are that you have chosen our office for your dental care and to explain our office policies.

Office Hours

  • Monday & Tuesday: 9:00am - 6:00pm
  • Wednesday & Thursday: 7:00am - 5:00pm
  • Friday: 7:00am - 2:00pm

Treatment

It is our intent to offer a comprehensive, modern program of dental care. Your initial visit will include a complete oral examination, cleaning, and diagnostic x-rays where indicated. A treatment plan will then be proposed with an estimate of costs you will incur. Upon completion of initial treatment, you will be put on a regular recall schedule, which must be followed to maintain your status as a legal patient of record. We cannot see you on an emergency basis only.

Besides, prevention is the key to a healthy mouth.

Appointments

Appointment times are reserved especially for you; therefore it is important that you be on time. We try very hard to maintain our schedule and even a ten-minute delay will cause a disruption to the treatment of the next patient. Arrival too much past your scheduled time may necessitate that you be assessed a late fee and your appointment rescheduled. On occasions when we are unavoidably detained, we ask your kind indulgence. It is our aim to give each patient our utmost attention and we need your help to stay on schedule. Thanks.

Frequently in treating patients under 18, we find it necessary to speak to a parent. Please try to accompany your child to his appointment or let him know where you can be reached at that time.

In an effort to keep fees as reasonable as possible and to try to accommodate your desired appointment times, it is imperative that any appointment changes be made at least 24 office hours in advance. Please note we are closed on Thursday; therefore, it cannot be counted as notice time. A $75 fee will be assessed for abuse of this policy. Further abuse will result in your name being removed from our active records and placed on a waiting list for future appointments.

Every attempt on our part is made to remind you of upcoming appointments. This is a courtesy we extent to our patients. If we are unable to reach you, we apologize, but it is still your responsibility to keep your appointments.

Insurance

Insurance coverage is an agreement between you and your employer. It is your responsibility to know what is and what is not covered. Again, as a courtesy to you, we will try to help you with this and to submit your claims two times if necessary. After that, we will give you a copy of the claim in question so that you can contact your insurance company. At this point, payment, including any finance charges, becomes your responsibility. This, too, has become necessary to try to keep our fees reasonable.

Please apprise the receptionist of any changes in your coverage as soon as you become aware of them. If you have an 18 year-old full-time student, we need to include where they go to school on the insurance form, so please provide us with this information.

Financial Terms

Payment is expected at the time of treatment. We gladly accept Master Card or Visa, in addition to cash or your personal check. If insured, you will be given our best estimate of your portion of the charges. Larger balances can be extended to several payments, with full payment due in 90 days. Orthodontic cases are billed over a twelve-month period.

Balances greater than 90 days will incur a finance charge of 1.5%. This balance may also be turned over to a collection agency, unless you have made payment arrangements with the office manager. If it becomes necessary to involve a third party for collection, you will be assessed our costs for this, including attorney fees, which may add 30% to your balance.

Consistent monthly payments are required. Failure to comply may result in a rebilling fee for every month you fail to make a payment. Your debt affects every patient’s costs, as it increases our overhead, and we feel it only fair for you to assume this cost.

While divorce cases are personally and financially unfortunate, we are unable to step in the middle or to wait for such cases to be decided legally. The person designated as responsible party will be responsible for payment and for contacting the former spouse.

Your cooperation with these policies will enable us to treat you as well and as cost-efficiently as possible and we greatly appreciate your compliance. It is our mission to treat you as we would one of our own family members, because to us, that is what you are. If there is anything that you feel we can do to improve our service to you, please do not hesitate to let us know. We look forward to a long, healthy relationship with you. New Payment Terms Starting March 1, 2023

In light of recent changes, Kewaskum Family Dentistry will be instituting some new payment terms starting March 1, 2023:

  • a 3.5% convenience fee will apply to all debit and credit card transactions

  • there will be a $35 processing fee for any check returned unpaid by the bank for any reason.*

*This is a banking fee the office will not take responsibility for.

Please make note of these terms for any upcoming appointments you may have so you can be prepared ahead of time with your preferred payment method.

We would also like to remind you that, besides debit and credit cards, we accept cash and checks for all of our services. If you are one of our patients without dental insurance, please ask us about your payment options before your next cleaning or treatment. In certain cases, you may be eligible for a discount when you pay in cash or with a check.
