Therapist sending the order? Medicare (CMS) clarified in 2026 that the lymphedema diagnosis must be documented by the treating practitioner (MD, DO, NP, PA or CNS) in their own progress note. Therapist notes can’t be used for the diagnosis, even if the provider co-signs them, so please upload the provider’s note too.
The patient also needs to sign our consent. They can sign online here: Patient Consent. You can open it on a tablet in your clinic or send them the link.
List each item exactly as it appears in your progress note and on the prescription.
