Artwork Donations Form

Please fill out the form below to have your artwork submitted for approval. When we look at artwork to approve, we look for hope inspiring pieces that are both eye-catching and appropriate for all ages. Your artwork must be no larger than 3' (36") in its longest dimension. It must also be 2 dimensional and capable of being framed. Artwork must remain anonymous while in the office and therefore also cannot be signed with a full name.
