Custom Orthotic Insurance Benefits

Our experience indicates that in most cases, custom­-made foot orthotics are usually a non­-covered benefit by most insurance companies. As a result, your insurance company is likely to deny benefits and payment for these devices. In most instances, payment for foot orthotics becomes the financial responsibility of the patient.

We recommend that you, as the patient, contact your insurance company to verify orthotic coverage. Insurance carriers will require the following coding information when you contact them:

Code for custom­ foot orthotics: L3000

Cost of orthotics: $ 390

Once you have contacted your insurance company, please return this form to our office. Any scans or cast impressions that have been performed will be placed on hold until this form is returned or a deposit has been made.
