FILLING OUT YOUR INTAKE FORM

Step 1

Download the form before you fill it in. If you type into the preview window that opens when you click the link, your answers won't save.

Step 2 - Fill Out The Form

On a Mac — open the downloaded file in Preview or Adobe Reader, type in theboxes, then File → Save (Command + S).
On Windows — open it in Adobe Reader (free at get.adobe.com/reader), type inthe boxes, then File → Save (Ctrl + S).
In Chrome or Edge — you can fill it in right in the browser, but you have toclick the download icon in the top right when you're done, or your answersare lost.
Do You Prefer paper? Print it fill it in with a pen, then scan the pages or photograph each one and upload the images. That works just as well.
Step 3

Upload it using the secure link below. You can upload several files at once — just make sure every page is there.
*If you're coming as a couple, each of you fills out your own copy, on your own.
