Please ensure your spreadsheet contains any of the following credentials which apply:

Domain/Server Admin (including service accounts like users for scan to folder)
Local Administrator (PC/Server)
Out-of-Band Management (iLO/iDRAC/IPMI/IKMP)
VMWare (ESXi and or vCenter)
NAS and or SAN
Firewalls/Routers
Managed Switches
Wireless Controller and or Access Points
Wireless SSIDs and Passwords
Backup Systems/Services
Email Security (SPAM filter, encryption, other email security services)

At a bare minimum we require a domain/server admin password for server environments. For the most thorough assessment review, please include as many of the following additional credentials which apply:

Local Administrator (PC/Server)
Out-of-Band Management (iLO/iDRAC/IPMI/IKMP)
VMWare (ESXi and or vCenter)
NAS and or SAN
Firewalls/Routers
Managed Switches
Wireless Controller and or Access Points

NOTE: All information provided in this form is encrypted in-transit and at rest. All submissions are protected by multi-factor authentication. By submitting credentials in this form, you authorize Access Systems to utilize the credentials for the purposes of the Network Assessment process. Should you choose to engage Access Systems for IT service, these credentials will be added to our secure customer documentation system. Should you choose not to engage with us for ongoing services, submissions will be expunged from our system either after one year or at your request, whichever happens first.
