Office Policies

Please correct the errors described below.

APPOINTMENTS: Office visits are by appointment only. It is the responsibility of the patient to schedule or cancel appointments. You will be responsible for the full fee for appointments not cancelled 24 hours in advance. Payment in full is due at the time of service.

EMERGENCIES: If an emergency arises call the office and advise the receptionist of your needs. If available your doctor will respond as soon as possible. If your doctor is unavailable then the "doctor on call" will be notified. If you don't get a response within a reasonable period of time, please call back. If the emergency arises outside of regular office hours call the medical exchange at 458-1121.

PHONE CALLS: Please call during telephone hours (Monday through Thursday 10:00 a.m. to 4: p.m.). There may be a charge for time spent on phone calls. There is no charge for calls related to medication problems.

MEDICATION REFILLS: Prescriptions with refills are written at scheduled appointments. It is the responsibility of the patient to schedule appointments before prescriptions expire. If your prescription has expired please call and schedule an appointment. There is a $10 charge for all prescriptions provided outside of scheduled appointments.

INSURANCE: Please understand that you are responsible for payment. For private insurance we will provide you with a receipt to allow you to file your claim. This office will not bill your insurance company. This office does not accept Medicare or Medicaid insurance.

There is a $35 fee for insurance prior authorization calls or a $10 fee for form completion.

ACCOUNTS: Payment in full is due at the time of service.

Time spent on correspondence is billed.
