Thank you for choosing 46 Dental P.C., to serve your dental care needs. We provide high quality dental care to our patients and are committed to providing treatments in a safe environment. Please note that payment for all services rendered is due on the day of service, unless prior arrangement have been made. For the convenience of our patients, we accept cash, checks, credit card (Mastercard, Visa, Discover, American Express) and care credit.

We offer 6 or 12 months differed interest payment (for charges over $200) with credit approval through an outside finance company (care credit - credit approval required).

Dental Insurance

As a courtesy, we will submit a dental claim to your dental insurance and receive a reimbursement from the insurance company for available benefits. As most insurance companies plan coverage may provide different reimbursement levels for different dental procedures, it is possible that 1005 coverage may not be possible for all dental treatment expenses. The final reimbursement depends on the deductible, co-insurance, and specific limitations provided by your insurance company plan, At our office, we carefully calculate your co-payments for each specific treatment. In certain situations, we submit pre-treatment estimates to the insurance company to find out available benefits. However, the actual payment made by the insurance company may differ from the estimate provided by the insurance company. Please understand that your dental insurance is a contract between the patient and the insurance company and not between the dentist and the insurance company. We will submit claims to your insurance company for reimbursement for provided services at no additional charge to the patient. Ultimately, the patient is responsible for the remaining balance of the treatment provided. You are responsible for notifying the office immediately if there are any changes or termination to your insurance coverage.

Appointments and Cancellations

We reserve a specific time for you when you make an appointment with our office. We request you to come for your appointment in time also. We will try our best to see you on time and utilize the time reserved for you in providing dental exam/consultation/treatment to you.

If you are unable to keep an appointment, please give us 48 hours' notice, so that your appointment can be rescheduled. Missed appointments or cancellations less than 24 hours are subject to a $50 (fifty-dollar) charge. This missed appointment charge is not covered by insurance. Missed/Cancelled appointments not only cause a delay in your treatment but also compromise your dental health and treatment plan outcome. In case if you are running late for your appointment, please call us and let us know so we can make necessary adjustments to our schedule or reschedule your appointment for a later time and date. We provide appointment reminders via phone calls, text messages, or e-mail. Patients are required to confirm their appointments and communicate with us if any changes are needed. If the patient does not confirm the appointment, we will assume that you will NOT be keeping your appointment and your time will be given to another patient. In case you have multiple missed appointments, 3 or more, we may advise dismissal from the practice. In this case, we will notify you, via registered/certified letter at your address of our record.

Delinquent Accounts

Delinquent accounts over 90 days, will have to be turned over to a credit reporting agency. Please let us know if you have any questions or concerns.
