Immediate Denture Consent Form

Please correct the errors described below.

Immediate dentures are inserted in the mouth the same day the remaining front teeth are extracted. The benefit of an immediate denture is that you will not have to go home without teeth. A disadvantage of immediate dentures is that with existing teeth in the way, we never get to see how the new dentures fit, look, or function. We do not know how your body will heal. The gums and bone undergo great change during the healing process and this adversely affects the fit of immediate dentures. The fit and appearance of immediate dentures is not ideal.

The following complications may occur:

  • Bone spicules or excessive bone may be present following multiple extractions. In this situation, additional surgery may be needed. There is an additional fee for these procedures.
  • Weight loss may occur due to soreness and difficulty chewing during the healing process.
  • Difficulty speaking and/or eating
  • Food collecting under dentures
  • Loose dentures
  • Lack of retention
  • Need for adhesive
  • Excess saliva – this typically returns to normal after some time
  • Functional problems
  • Inability to adjust to wearing dentures due to gagging, unnatural feeling, discomfort, or other issues
  • Altered taste
  • Denture teeth may be straight, but in the mouth may appear uneven.

Appointments:

  • Expect to have 4 to 6 appointments during denture fabrication.
  • For 3 to 5 weeks following delivery of your denture(s), you will require follow up appointments to adjust the denture as your mouth heals. It is normal for your denture to loosen as your mouth heals. The ridges (bone and gums) that held your teeth in before will shrink and change shape.
  • At the end of one month, if your denture is too loose to function properly, a temporary healing reline will be placed to refit the denture.
  • Complete healing and reshaping of the ridge can take from 6 months to 1 year. At this point, the dentures are evaluated and may need a reline, rebase, or a new set may need to be made.

Cost:

  • The cost of an immediate denture includes the denture, denture adjustments, and temporary healing relines needed for the first three months following delivery. After this, there may be a charge for adjustments.
  • At the end of 12 months, the denture fit will be evaluated. Most patients need to have a hard reline at this time. The dentures will need to be kept in the lab for one full day. There is a separate fee for this service.
  • Some patients will require a new set of dentures to be made. There is a separate fee for this.
  • Yearly exam, oral cancer screening, cleaning of the dentures. There is a separate fee for this service.

Expectations:

  • I cannot guarantee that a patient will be able to successfully and comfortably wear a well constructed denture. I pledge to do the best job possible given the oral condition that exists in your mouth.
  • Dentures are not a replacement for teeth, but an appliance to help someone with no teeth. Dentures are not real teeth. Chewing efficiency will be much less than with real teeth.
  • The average denture requires a reline or remake every 4 to 5 years.
  • Bone resorption will continue throughout your lifetime, making subsequent denture construction more difficult, less satisfying, and less comfortable than your previous denture experience.

I have been provided verbal and written information about immediate dentures and all of my questions have been answered to my satisfaction. I give permission to have my teeth extracted and an immediate denture fabricated.
